Prosthetic Rehabilitation in a Four-year-old Child with Severe Early Childhood Caries

Severe caries in childhood affects several teeth in both arches and occurs in children under 05 years of age. The lesion spreads and in many cases compromises the dental pulp.

Oral rehabilitation restores the different functions lost due to the destruction and absence of teeth. Thanks to the rehabilitation, a correct dental and maxillary development is ensured.

We share the case of a 4-year-old girl who presented coronal destruction in several teeth. She was rehabilitated with steel crowns, celluloid crowns, orthodontic treatment and removable partial dentures.

Step-by-step preparation of molars and primary incisors for Zirconia crowns

Zirconia crowns are a great esthetic, biocompatible and resistant alternative for dental rehabilitation of pediatric patients. They are used when the tooth has been affected by extensive caries or dental trauma.

The greatest benefit of Zirconia crowns is aesthetics, compared to stainless steel crowns. At the time of preparation, a series of considerations must be taken by the pediatric dentist.

The Hall Technique: Manual for the management of primary molar caries

Hall Technique

Dental caries is an infectious process that affects both children and adults. When caries affects a large amount of dental tissue, a procedure is required to ensure the permanence of the primary tooth in the oral cavity.

The Hall technique is used on primary molars affected by caries, using preformed steel crowns. The effectiveness of this technique is proven, but a careful evaluation by the pediatric dentist is required.

Hall Technique for Stainless Steel Crown in 3 easy steps

The Hall technique is an alternative to the treatment of extensive cavities that have affected a large amount of dental tissue in the primary teeth. This technique consists of placing a preformed metal crown without dental preparation.

Steel crowns are press-fitted on primary molars affected by extensive caries or when the enamel is defective. Thanks to the steel crowns, tooth loss and future occlusion problems are avoided.

Severe early childhood caries: an integral approach

It is considered early childhood caries, when the carious process affects one or more primary teeth in children under 06 years of age. This infection is of multifactorial etiology, transmissible and of rapid development.

Treatment will depend on the extent of the injuries and requires interdisciplinary management to achieve successful rehabilitation. In these cases the cooperation of the parents is necessary.

Preformed crowns for decayed primary molar teeth (Review)

Dental caries (tooth decay) affects around 60% to 90% of children globally (WHO Report 2003). It most commonly occurs in primary (baby) molar (back) teeth. If left unmanaged, dental caries will progress to give pain and infection, the consequences of which are unnecessary suffering, and lost days at school (Gift 1992).

There is evidence of a linear relationship between higher levels of caries and anthropometric outcomes (height, weight and body mass index (BMI)) (Alkarimi 2014).

Untreated dental caries has an adverse effect on children’s ability to grow and thrive (Sheiham 2006), and in underweight children, extraction of severely carious primary teeth can improve weight gain (Monse 2012).

Teeth undergo a constant process of demineralisation (caused by the acids and enzymes produced by cariogenic bacteria in the dental biofilm (plaque)) and remineralisation (from protective factors such as fluoride and salivary components).

Oral Medicine

Dental caries occurs when the rate of demineralisation is greater than that of remineralisation. When dissolution of enamel and dentine leads to cavitation (a hole in the tooth), the carious lesion can no longer be cleaned and it becomes more difficult to arrest the caries process as the biofilm becomes more sheltered, which favours the cariogenic bacteria (Fejerskov 2015).
